The Heart Failure Management program runs concurrently with the Pulmonary Rehabilitation Program
The program guides, supports and assists people who have been diagnosed with heart failure to learn about their condition and become aware of ways to better manage it in partnership with their doctor. It is based on the most current Australian clinical practice guidelines for heart failure – Guidelines on the Contemporary Management of the Patient with Chronic Heart Failure in Australia (2002).
The WDHS Heart Failure Management program is available to any person with heart failure. This includes cardiomyopathy. To take part in the Managing Heart Failure Program, a referral is required from a doctor or referring hospital, or direct contact may be made with the service. Eligibility is then determined during a pre-program assessment. Participants are followed up at program exit, and at 3 and 9 months post program completion.
Information sessions have a self-management focus, with topics including:
- Managing your symptoms
- Your medications explained
- Energy conservation
- Action planning
- Suggestions for good nutrition
- Managing work, rest & leisure
- Benefits of exercise…and more
Location
The program is held in the Chalet Physiotherapy Centre situated next to the Allied Health Centre, Tyers Street, Hamilton. We ask that participants in the program wear comfortable clothing and appropriate footwear for light activity.
Transport assistance can be arranged through the program coordinator for participants who are unable to attend the program due to transport difficulties.
Hours of operation
The program is held each Monday & Thursday from 1.30pm – 3.30pm for 8 - weeks. Four to five programs are held each year.
