Hospital Admission Risk Program

What is HARP?

HARP stands for Hospital Admission Risk Program. It is a program designed to provide the right care in the right setting, to reduce avoidable hospital admissions and Emergency Department presentations to contribute to better health outcomes for people in the Southern Grampians Shire.

Aims of the program

Assist people with chronic and complex conditions to stay healthy at home by:

  • Providing care coordination for clients
  • Providing information / education and support to clients and their carers
  • Liaising with the client their GP, carer, and service providers
  • Promoting self management through care planning
  • Improving client’s access to services / treatment options

Providing ongoing support and monitoring to ensure early intervention with disease exacerbations or decline

  • Provision of emergency care plan/contacts to clients and carers
  • Management of medication issues, in collaboration with GPs and pharmacists.

Who can benefit from the program?

People over 65 years, (Aboriginal & Torres Strait Islanders over 45 years) who reside within the Shire of Southern Grampians and fit at least one of the following criteria

  • People with a diagnosis of chronic heart disease or chronic respiratory disease
  • People with complex needs requiring coordinated care
  • People at risk of avoidable admission / presentation to the Emergency Department

 What will the program involve?

Thorough assessment - including health conditions and how they impact on the client’s life

Support from HARP clinicians - to develop the client’s skills and knowledge to allow them to best manage their condition.  This may include home visits, support to access other services / specialists and developing a plan with the client of how to manage their condition best.

The HARP team is flexible and can vary the level of support they provide depending on how the client is managing their condition at the time.

Referral to HARP

Referral can be from a medical practitioner or other health care professional, or people may contact the HARP Manager directly.

Fees

There is no client fee for this service however a cost may apply if aids, equipment or other services are required.
