District Nursing Service

What is District Nursing?

District nursing is professional nursing care provided by Registered Nurses, it’s offered to people of all ages with any medical problem or disability who choose
to be cared for in their home.
Our aim is to encourage clients to achieve a level of independence that will allow them to remain at home.
Also our role is to support to the carers who play a vital role in maintaining the client at home.

Referrals

Referrals can be accepted from all sources. A written referral from the doctor is necessary if any medical treatment is required.

 

Eligibility for Service

Eligibility is subject to an assessment by a District Nurse following a referral. Following this assessment a plan of care will be developed and visiting days established.


Area Serviced

The city of Hamilton and an 11 km boundary also the towns of Dunkeld, Cavendish, Penshurst, Caramut & surrounds.
Care for people in outlining areas who do not fall into the designated boundaries can be negotiated with adjoining District Nursing Services.

 

Hours of Service
The city of Hamilton is serviced 7 days per week from 8am to 9pm
Dunkeld, Cavendish, Penshurst and Caramut – a part time, weekday service is offered to these areas.

A wide range of services is offered eg:

  • Wound care
  • Continence Care
  • Leg Ulcer Management
  • Diabetes Management
  • Stomal Care
  • Palliative Care
  • Paediatric Care
  • Breast Care Support
  • Medication Management
  • IV Therapy including IV administration of drugs
  • Central Venous Catheters including PICCS, Ports and Hickmans
  • Home Oxygen Therapy

Cost to Client
Fees are in accordance with Government Policy. Service will not be denied to clients experiencing financial hardship.
