Cardiac Rehabilitation Outpatient program

Services

Cardiac rehabilitation describes all measures used to help people with heart disease return to an active and satisfying life and to prevent recurrence of cardiac events. It is an organised approach to achieving specific aims and should be integrated into the routine management of all patients. It is based on best practice guidelines set out by the National Heart Foundation of Australia.

The WDHS outpatient Cardiac Rehabilitation program is available to any person with cardiovascular disease. This includes:

  • Following a heart attack, or
  • Prior to &  following heart surgery,
  • Coronary angiography or stenting,
  • People with angina or other cardiac condition.

To take part in the Cardiac Rehabilitation Program a referral is required from a doctor or referring hospital, or direct contact may be made with the service. Inpatient referrals are accepted via Trakcare, and external referrals are accepted via Service Coordination Tools or by letter or fax.

Participants have a pre-program assessment and are followed up at program exit, and at 3 and 9 months post program completion.

Information sessions have a self-management focus, with topics including:

  • Understanding heart disease and it’s risk factors
  • Your medications explained
  • Healthy eating for the heart
  • Energy conservation
  • Diabetes
  • Returning to work
  • Exploring the myths of physical activity
  • Supermarket tour…and more

We ask that participants in the program wear comfortable clothing and appropriate footwear for light activity. Carers and partners are more than welcome to come along too.

Transport assistance can be arranged through the program coordinator for participants who are unable to attend the program due to transport difficulties. This is particularly useful for clients who have driving restrictions following hospitalisation.

Hours of operation

The program is held each Wednesday from 2pm - 4pm for 9-weeks. The program runs continuously throughout the year so participants can enter at almost any time following assessment.

Location

The Cardiac Rehabilitation program is held in the Chalet Physiotherapy Centre situated next to the Hamilton House Allied health Centre, Tyers Street, Hamilton.
